| W9 FORMS Regardless of age or amount of money won, you are required to submit a completed W9 to the show office to collect earnings. If you have moved or changed your last name since the last time you submitted a form, submit a new form to the office. Forms are available at the show office or can print and complete one from the link above and deliver to show office, email to goforbrokebarrelrace@gmail.com PAYOUT Friday race checks are available to p/u at the show office on Saturday (time TBA) Saturday & Sunday checks are available to pick-up approx 30min after the days race. Checks not picked up at the show will be mailed to the address in your SADDLEBOOK account. Login to your SB account and confirm your address is correct. Checks not picked up at the event are mailed 5-7 business days after the last day of the event. We will no longer re-issue checks without a re-issue fee of $40. Checks not cashed 60 days after issue date or checks that aren’t picked up (due to w9 not being turned in) will be void after 60 days and monies put back into the year end awards fund. PLEASE make sure you have a current w9 on file with GO FOR BROKE PRODUCTIONS to collect your winnings Any payout made in CASH (pee wee or Jackpot Junkie events) must be picked up at the show. It will not be held or mailed. Cash payouts not picked up within 90 days will be applied to the GFB season awards fund. |  |

Event Disclosure & Terms of Entry
By entering this event, you (the rider/participant) agree to the following:
1. Event Participation
- You are responsible for reading, understanding, and following all event rules and regulations.
- Participation in equine or domesticated animal events involves inherent risks that you voluntarily accept.
- Liability: Go For Broke Productions (GFBP), its staff, and associates are not liable for any accident, injury, theft, death, or property damage while attending, competing, camping, or spectating.
- GFBP reserves the right to refuse entry to any participant at any time, for any reason.
- By entering, you waive all claims against GFBP and its affiliates for damages, injuries, or losses incurred during the event.
2. Iowa Domesticated Animal Law Notice
Under Iowa Code Chapter 673 (Domesticated Animal Activities):
- A “domesticated animal professional,” event sponsor, or exhibitor is not liable for damages, injury, or death arising from the inherent risks of domesticated animal activities.
- Exceptions include cases involving intentional or reckless acts; use of defective equipment or tack; failure to warn of a known dangerous latent condition; or injury to spectators who are in places where they would not reasonably expect such activities.
- Required notices: A warning sign must be posted in visible locations on properties where such activities occur, and, if there is a written contract, the contract must include a notice of assumed risk.
3. Media Disclosure
- By entering, you consent to being contacted by email, text, or phone about event updates, results, promotions, or feedback.
- You may opt-out at any time by replying “STOP.”
- You consent to photos and videos taken during the event to be used for marketing and promotional purposes (social media, advertising, etc.).
4. Terms & Conditions
- Pricing Errors: GFBP reserves the right to refuse the sale of any item listed incorrectly due to website or programming errors. Efforts will be made to correct errors promptly.
- Modification of Terms: GFBP may update or modify these Terms & Conditions at any time without prior notice. Updates take effect immediately upon posting online or at the event office.
